Nearly every 500EXT issue traces back to the ribbon cable or card-edge connector going in backward or misaligned at one end. The sections below are ordered by likelihood, so work down from the top.

General resources

No continuity, or connections seem reversed at one end

Check the IDC ribbon cable and card-edge connector orientation, paying close attention to pin 1 alignment on both ends. Soldering the ribbon cable or connector backward is the most common cause of a 500EXT that doesn't pass signal correctly. Test continuity between the boards pin-by-pin with a multimeter to confirm.

Partial or no voltage on the ±16V or 48V rails when testing a module through the 500EXT

This almost always traces back to the same soldering or orientation mistake as Section 1 rather than a defective extender board. Go to Section 1 and check the ribbon cable and connector orientation.
